The HOPE project aims to develop home care as a fully integrated and equal component of health and social services in the Czech Republic. It responds to the country’s changing demographic structure and the increasing pressure on the healthcare system.

The project focuses on several key pillars: enhancing the professional competencies of home care workers, supporting informal caregivers and introducing modern tools that can significantly improve the efficiency of care delivery. Particular emphasis is placed on areas such as case management, telemedicine, use of new technologies, quality assessment and patient categorization based on individual needs.

An essential part of the project is the development and updating of educational programmes designed to broaden professional knowledge and improve navigation within the care system. The project aims to support not only healthcare professionals but also family members who often find themselves suddenly in caregiving roles without prior experience.

HOPE seeks to create a supportive system tailored to the real needs of field practice. A key activity of the project is the establishment of a reference network of home care providers across all eight NUTS 2 regions of the Czech Republic. These partners will pilot new approaches and test proposed methodologies in real-world settings.

The project involves a wide range of stakeholders, including healthcare providers, professional associations, educational institutions and an international partner from Switzerland. The Swiss experience with integrated home care systems will serve as a valuable inspiration for designing a functional and sustainable model adapted to Czech conditions.

The HOPE project thus represents a crucial step towards modern, efficient and long-term sustainable home care in the Czech Republic.
